It is worth noting that in any live-action work it is incredibly unlikely that a live grenade will be present at any stage of filming due to safety concerns: grenades held by actors are essentially always dummy / training variants or props, while the effects for grenade explosions are often made big and fiery using petrochemicals as opposed to real ones which tend to be rather less flashy. Even smoke grenades that are used on-screen will typically not be the effect of a live weapon being set off, as military smoke compounds tend to be toxic as a tradeoff for greater effectiveness.

The blast radius and penetrating ability of a fragmentation grenade (with red-hot fragments often able to penetrate every internal wall of a building and sometimes even set it on fire for good measure) are usually minimised in fiction, with even an obstruction as insubstantial as a wooden door sometimes being enough to offer complete protection from a frag grenade. The kinetic effect of the blast on the other hand is often hugely exaggerated, throwing people into the air in extremely dramatic ways without creating any visible wounds.
