The OTs-14-4A is the most common variant of the rifle, chambered in 9x39mm. A slightly curved magazine was developed for the OTs-14-4A, but the gun can also take straight magazines originally introduced for the 9A-91 compact assault rifle. This version of the Groza holds 20 rounds.

Four variants of the OTs-14-4A are known: The OTs-14-4A comes equipped with a GP-25 or GP-30 grenade launcher. When a grenade launcher is installed, the combined rifle and grenade launcher is operated with a single trigger. This is the first variant, which was developed to be an assault rifle / grenade launcher weapon system. The OTs-14-4A-01 is fitted with a vertical foregrip, attached directly onto the barrel. The OTs-14-4A-02 has no attachment. The OTs-14-4A-03 is the last variant, which is equipped with a sound suppressor and optional scope.
